Which risk adjustment formula is sustainable and efficient in a managed competition system for social health insurance? Can risk selection be mitigated by mandatory high risk pooling? How does regulation affect the exposure of insurers to risk? Which payment schemes for managed care organizations set strong incentives both for efficiency in health care delivery and solidarity with high risks?
