Care Management

Care Management provides valuable support at times when we are dealing with health challenges. Whether that's coming out of hospital, coping with chronic illness or receiving a mental health diagnosis, a personal point of contact will guide you through the complexities of the healthcare system.

When does Care Management make sense?

Care Management offers valuable support in dealing with health-related and personal challenges, creating order and structure during periods that are difficult for your health.

It is particularly helpful in the following situations:

  • After a long inpatient stay, to make leaving hospital and organising transport, therapy and home help easier.
  • In complex health situations involving multiple parties, or where no support is available from the family or outpatient services.
  • For mental health diagnoses such as depression or burnout, to provide assistance with the next steps.
  • For chronic medical conditions, to find your way through the complexities of the healthcare system.

We'll check your details and your case before deciding whether we can offer you personal Care Management. We'll then get in touch with you to discuss the next steps.

Personal support

On joining the Care Management programme, you'll be assigned your own personal point of contact at CSS. They will advise you and accompany you along your treatment pathway. They'll help coordinate the best possible treatment and the next steps to be taken, such as with doctors, specialists or others (e.g. Pro Senectute). Your point of contact will also help you with administration and with any insurance questions or problems related to your case.

Frequently asked questions

No. Personal Care Management can give you a new perspective on your situation and provide you with input. It's therefore no substitute for a personal consultation, examination or diagnosis by a recognised doctor. Neither can it speed up any decisions to be taken by the doctor (e.g. referrals or imaging procedures). Responsibility for the medical management of the case remains with the medical professional (doctor). We can't assume any responsibility for the outcome of the treatment and disclaim all liability in this respect.

Care Management is available to anyone insured with CSS. A personal point of contact will accompany, support and advise people struggling with difficult health situations.

Personal Care Management does not constitute insurance advice. No information will be provided about current commitments to provide cover, bills, insurance models or premium reductions. Questions relating to addiction diagnoses, second opinions and assessments of medical diagnoses also can't be answered by the Care Management service.
